With the emergence of the COVID-19 pandemic, health equity gaps across the U.S. were exposed, bringing the need for quality healthcare for vulnerable populations to the forefront and many healthcare systems turning to digital tools and telehealth services to maintain access to care. Among those health systems was Erie Family Health Centers. The Erie team leveraged new technological tools to continue providing access to quality healthcare to underserved communities. In response to its strong effort throughout the pandemic, Erie was recently awarded a $2M grant from the Health Resources and Services Administration (HRSA) to further its goal of providing even greater access to quality healthcare for all. In March 2022, Erie transitioned to using Epic, an electronic health record system, to allow for more robust data management, care coordination and efficient communication among providers and patients. Additionally, patients can now download the MyChart patient portal app as another resource to further enhance accessibility to healthcare as it allows patients to communicate with the Erie care team directly, get referrals and test results, update contact information and easily access their medical history.  To learn more about available resources, please visit https://www.eriefamilyhealth.org/patient-resources/ or call (312) 666-3494.
