Saint Anthony Hospital Receives $30K from Fry Foundation

Lawndale News Chicago's Bilingual Newspaper - SaludSaint Anthony Hospital, 19th Street and California Ave, has been awarded a $30,000 grant from the Lloyd A. Fry Foundation to implement systematic protocols with key referral sources to improve health outcomes among adults with diabetes. With the Fry Foundation’s support, Saint Anthony’s nurse-managed Center for Diabetes, Nutrition, Obesity, and Metabolism will add a bilingual care coordinator to its healthcare team to streamline referrals from the hospital’s community-based programs and other units inside the hospital, including the emergency department and ambulatory care clinic.

Saint Anthony sits on the border of the city’s North Lawndale and Little Village communities. North Lawndale has one of the highest rates of diabetes in Chicago, and Little Village has one of the city’s highest age-adjusted rates of diabetes-related hospitalizations. The Lloyd A. Fry Foundation is a private, independent, grant making organization. It supports organizations with the strength and commitment to address persistent problems of urban Chicago resulting from poverty, violence, ignorance, and despair.
